At East Slope Family Practice, we believe great healthcare starts with knowing the people behind the patients. We're a locally owned, independently operated family practice right here in Great Falls — built on the idea that when we care well for our patients, we're caring well for our neighbors.

We offer comprehensive primary care for every member of your family, from newborns to grandparents, all under one roof. Whether you're coming in for a routine wellness visit, managing a chronic condition, or need to be seen today, we're here for you.

Your health.
Your family.
Your community.

East Slope Family Practice

Our purpose is simple: to provide quality, compassionate, individualized primary healthcare to families in and around Great Falls — and to help our community live healthier, fuller lives.

We're not a corporate chain. We're your neighbors. Combined, our providers bring more than 30 years of experience in the local healthcare system, and we plan to be here for the long haul.

Our Mission

Jeremiah Watt, FNP, is a dedicated Family Nurse Practitioner with a strong background in emergency and critical care medicine. Prior to earning his FNP, Jeremiah served as both an ER and flight nurse in Great Falls, Montana and the greater Seattle, Washington area, gaining extensive experience in high-acuity and trauma care. His time in emergency medicine shaped his calm, confident approach and ability to think quickly in complex clinical situations.

He now provides patient-centered care for individuals and families, focusing on prevention, education, and practical solutions that fit real life.
He is committed to building strong relationships with his patients and helping them feel informed, respected, and empowered in their healthcare decisions.

Outside of medicine, Jeremiah is deeply rooted in Montana life. He and his wife, Brooke, are raising their three children - Keller, Miles, and Letti - with a love for the outdoors. He enjoys hunting, fishing, hiking and spending time with his family and their numerous critters.

Jeremiah is also passionate about mentoring young athletes. He coaches wrestling for the North Montana Wrestling Club, North Middle School, and C. M. Russell High School, investing in the development of both character and athletic skill in the next generation.

Jeremiah is proud to serve the community he calls home and is honored to care for families across Montana.

Kayla Boltz is a board-certified Family Nurse Practitioner certified through the American Association of Nurse Practitioners (AANP). She earned her Bachelor of Science in Nursing from Montana State University and completed her Master of Science in Nursing with a focus in Family Nurse Practice through the University of Cincinnati.

Kayla began her nursing career serving patients across multiple stages of care, including transitional care and hospice coordination, before moving into labor and delivery and women’s outpatient health. Through these experiences, she developed a strong appreciation for caring for women and families throughout the lifespan and recognized her desire to build long-term, relationship-based care.

Kayla’s approach blends evidence-based medicine with a thoughtful, individualized perspective. She has a strong interest in preventative care, metabolic health, and hormone evaluation, helping patients feel supported not only in managing illness but in improving overall wellness. She is committed to building lasting relationships with patients and providing dependable, accessible care close to home.

Outside of the clinic, Kayla enjoys spending time with her husband, their two children, and their two dogs. As a family, they love outdoor activities, camping, and helping on their family ranch.

She looks forward to continuing to partner with patients in their health journey and serving the community she calls home.

Our team

Katie grew up in Oregon, where her love of helping others led her from special education teaching into emergency medicine. She earned degrees in Paramedicine and Fire Science and built a career as a full-time paramedic before Montana became home in 2011 — opening the door to 13 remarkable years in air medical transport with Mercy Flight.

After starting a family, Katie joined East Slope Family Practice from day one. As Office Manager, she keeps things running smoothly and is known for her organization, adaptability, and sense of humor.

Outside of work, she's married to a hotshot firefighter and mom to two energetic kids. The family lives a true Montana life — skiing, hunting, hiking, fishing, and running a lively "funny farm" that's never short on activity or laughter.
