If you have questions about a specific medical procedure not listed, or would like to schedule an appointment, please contact us.
If you need to schedule an appointment as a new patient, we try to be flexible and can typically schedule new patients within the same week of requesting an appointment. Of course this changes depending on how busy we are but we try to get everyone in as soon as possible for evaluation.
For established patients, same day appointments at the clinic are often possible but may be with a provider other than your primary provider.
When you come to East Slope Family Practice as a new patient, please plan to arrive at the clinic 15-20 minutes prior to your appointment time to complete the required medical forms.
It is requested that you bring your insurance card if you are planning on utilizing insurance for your visit. Co-payment is expected at time of service as well. We accept cash, checks and credit cards for payment.
Upon arriving to East Slope Family Practice, you will be checked in and asked to fill out any required paperwork for your visit by our administrative assistant. Very rarely is there a wait time between this process and seeing a provider in the clinic.
Our Nurse Practitioners will complete all aspects of your visit, allowing them to get to know you better and provide more comprehensive care. For example, they will check your blood pressure and if you need lab testing done, they will draw your blood in most situations. They will ask questions about your medical history and concerns.
Both providers at East Slope Family Practice work hard to make your visit as comfortable as possible.
We'd love to hear from you! Email or call our clinic staff with any questions you have. If you have a medical question, please contact us to make an appointment.
